Drug abuse is an important sociomedical problem in large metropolitan areas. Drug addicts represent a group with particularities, since they hesitate to seek medical care and often refuse hospitalization. Therefore, there is a scarcity of data on drug abuse-related calls. The burden imposed by such calls on emergency health services has not been evaluated in detail. The objectives of this study are to: (1) assess the profile of drug abuse-related calls in a large European metropolis, including the spatiotemporal distribution, as well as the frequency and variability of cancellations; and (2) evaluate the mobilization of emergency prehospital care services in response to the calls. In 2005, the Hellenic National Centre for Emergency Care received 5,836 emergency drug abuse-related calls pertaining to the metropolitan area of Athens, Greece. The analysis focused on: (1) spatiotemporal features of calls/cases; (2) step-by-step cancellation rates in the mobilization of ambulances or other means (mobile intensive care units, specially equipped motorcycles, and super-mini city cars); and (3) response time of the mobilized means. Pearson's chi-square, goodness-of-fit chi-square, and the Kruskal-Wallis tests were used as appropriate. Drug abuse-related calls represented 2% of all emergency calls. Only one-third of these cases were transported to the Accident and Emergency Departments of area hospitals. A total of 9% of the calls were cancelled before transportation arrived; another 20% of victims could not be found when authorities arrived on-scene, and 36% of patients refused transport to the hospital. The cancellation rate is significantly higher in the evening and at night, as well as in summer and autumn. The major burden is imposed on the municipality of Athens (67% of all calls). Drug abuse-related calls represent a significant load for emergency medical services in metropolitan Athens. However, a relatively small percentage of the drug addicts finally are transported to the hospital. Appropriately equipped motorcycles seem to be an effective means for the prehospital management of drug-abuse cases.

To measure the patient access time interval and characterize its contribution to the total emergency medical services (EMS) response time interval; to compare the patient access time intervals for patients located three or more floors above ground with those less than three floors above or below ground, and specifically in the apartment subgroup; and to identify barriers that significantly impede EMS access to patients in high-rise apartments. An observational study of all patients treated by an emergency medical technician paramedics (EMT-P) crew was conducted using a trained independent observer to collect time intervals and identify potential barriers to access. Of 118 observed calls, 25 (21%) originated from patients three or more floors above ground. The overall median and 90th percentile (95% confidence interval) patient access time intervals were 1.61 (1.27, 1.91) and 3.47 (3.08, 4.05) minutes, respectively. The median interval was 2.73 (2.22, 3.03) minutes among calls from patients located three or more stories above ground compared with 1.25 (1.07, 1.55) minutes among those at lower levels. The patient access time interval represented 23.5% of the total EMS response time interval among calls originating less than three floors above or below ground and 32.2% of those located three or more stories above ground. The most frequently encountered barriers to access included security code entry requirements, lack of directional signs, and inability to fit the stretcher into the elevator. The patient access time interval is significantly long and represents a substantial component of the total EMS response time interval, especially among ambulance calls originating three or more floors above ground. A number of barriers appear to contribute to delayed paramedic access.

Obtaining accurate information from a 112 caller is key to correct tasking of Helicopter Emergency Medical Services (HEMS). Being able to view the incident scene via video from a mobile phone may assist HEMS dispatch by providing more accurate information such as mechanism of injury and/or injuries sustained. The objective of this study is to describe the acceptability and feasibility of using live video footage from the mobile phone of a 112 caller as an HEMS dispatch aid.

In prehospital emergency medical services, one of the key factors in the successful delivery of appropriate care is the efficient management and supervision of the area's emergency medical services units. Paramedic field supervisors have an important role in this task. One of the key factors in the daily work of paramedic field supervisors is ensuring that they have enough of the right type of information when co-operating with other authorities and making decisions. However, a gap in information sharing still exists especially due to information overload. The aim of this study was to find out what type of critical information paramedic field supervisors need during multi-authority missions in order to manage their emergency medical services area successfully. The study also investigated both the flow of information, and interactions with the paramedic field supervisors and the differences that occur depending on the incident type.
Interactive cognitive artifacts for enhancing situation awareness of incident commanders in mass casualty incidents

In mass casualty incidents, several members of Emergency Medical Services have to take actions in the field in order to cope with many injured or sick people. Incident commanders are responsible for managing operations, guiding rescue forces and applying resources appropriately under extraordinary circumstances. Data required for situation assessment, projection of developments and decision making are gathered by many different emergency physicians and paramedics. They are shared by numerous face-to-face talks, radio and phone calls as well as with the aid of paper-based forms and notepaper. While these tools and means of communication support flexible modes of operation, they often lead to deficient awareness of the situation. Due to temporal delays, poor handwriting and incomplete data, information sharing in the field is hampered, delayed and faulty. Compared to established paper-based artifacts, interactive cognitive artifacts might improve the situations by exchanging and visualizing data in real-time. However, because of users’ workload and working conditions, designing mobile computer-based tools and systems for this context of use is not only a technical but also a usability challenge. Based on the results of a two-year user-centered system design project in cooperation with German Emergency Medical Services, we discuss currently used and interactive cognitive artifacts for incident commanders. Challenges and approaches for successful user interface and interaction design are described and future work is outlined.
